1. I think that if something can happen, it will. So without whitelisting an accident with an unpleasant upload will happen sometime. It is just a matter of estimating the risk against the fuss. Cleaning up unwanted uploads is less work than maintaining the whitelist? (The whitelist has also a positive psychological effect.))
2. In my experience, the whitelisting is not an important hurdle, but the other requirements and lack of examples/documentation are.
(The Pattypan uploader lacks hurdles, whitelisting and a number of extra features of GWT. Interesting to see how long it takes for abuse to occur.)
So i would say, keep the whitelist but then i'm not an expert.

The whitelist is not for the community. Its out of concerns of malicious webservers attacking wmf servers (Personally I think that's a little paranoid, but that's the way it is). There's a bug about this somewhere in phabricator

Users have to apply for the right and then they have to whitelist the domain, too complicated.
For example https://tools.wmflabs.org/url2commons/ has no restriction at all (as far i know), at least not a domain whitelist.
