Yeah, this is painful. Have a look at the red box in the compatibility
section on this page:

If you install the skin correctly for your version of MediaWiki the
icons should show.

easiest workaround is to rename the Foreground skin folder to foreground and make the
same changes in LocalSettings.php
Sorry, I haven't had the time to chase down where the lowercase name is set.
