I totally agree with you Chris. However, I think the issue here is not whether or not we should have a name field, but rather the fact that I have access to the names which jeopardises the concept of privacy and the possibility of participants not opening up.

Greetings, the name field is quite an important part of this survey so as to prevent individuals from participating more than once since we currently lack a way of preventing such. However, these names shouldn't be revealed to anyone after the survey or when the results are being released. It is an important ethical rule when conducting a survey to protect the information given by your respondents. Then I think the whole idea of privacy doesn't make sense if this is the case.
Personally I'd prefer if the whole thing was made open including the names but if guys aren't comfortable with this, the name field should be removed.
