Check out http://www.mediawiki.org/wiki/Extension:SelectCategoryTagCloud
to see if that fits your purpose. Although it might be a little
overkill for what you want to accomplish. If you look at the source
code, it would be very easy to create a input box below the article
that lets you create a category for a page.

If you want to create a category without assigning it to a page at
all, can you please explain further why you would want to do that?
