A not so fantastic workaround that works is let the people who want an
account send an email to request an account to someone, a sysop or if
you have it your OTRS-queue.

The need to do;
1) check of there username is free
2) give a username
3) take the email address from the email and created the account and
pick   to do it by email.

I do it that why for people who can not make an account because the are
behind some proxy that I can not unblock or because the user is to
technical incompetent to do it thereself. Does not happen a lot.
