[teampractices] quarterly goals format - update?

Hi Greg -- when we discussed this in some meeting or other, we agreed that
it was too late for Q3 but we should try for Q4.

I'd get it over with -- according to my whiteboard schedule of doom draft
goals are due at the end of February (2/26)
