[teampractices] Critical Success Factors and Ideas for Improvement (survey)

I looked at the survey and some of the wording/grammar was disastrous! :-D In my defense, the list was typed out in real-time at the meeting, and this list came from my notes.

I tried to clean it up. But in a few cases, I forgot myself why I typed things out and was unable to come up with a better way of phrasing things. :-)
