[QA] New fatal errors on beta labs

I updated monitor_fatals.rb to report the name of every extension reporting
a fatal error in the file: https://gerrit.wikimedia.org/r/#/c/116146/
It will now send an email message that looks like

The file at /data/project/logs/fatal.log on the deployment cluster has a new
entry within the last twelve hours.

The extensions with fatal errors:

MobileFrontend
UniversalLanguageSelector
Wikidata
