I have reenabled the bw-noicon a day or two ago and it seems to be
working OK. At least I haven't seen the same issues anymore yet, so
let's assume for the moment it is fine and was caused by something else.

I have updated the queries to take advantage of the new building index
in the default mapnik map style in the osm svn repository. If you resync
with that, you will hopefully get them too.

The qai- and qa- remain disabled for now until they are fixed.

Finally, I have also replaced the "germany" style with the new one that
is also shown on openstreetmap.de. As it turns out to be quite different
to the previous germany style, the map might look a little odd until it
is through re-rendering everything.

I haven't commited all of these changes to the p_osm repository though yet.
