[teampractices] [ANY] Looking for live example of a Phabricator custom form

Thanks for the updates. It's my understanding that in order to get a custom
form for a team, it should be requested via the Phabricator project
<https://phabricator.wikimedia.org/tag/phabricator/>. Is this correct?
My hope was to make it easier for my teams to make tasks by autopopulating
certain things, like Projects that must always be tagged, or prefilling
information that is always required (such as repro steps for bugs). Is this
an appropriate use of forms?
