Remove .htaccess (if exists)

If still not work, change/add at the top of LocalSetting.php:
$wgScriptPath = "/w";
$wgArticlePath = "{$wgScript}/$1";

Under the Murphy Law, the steps provided above may not fix that
problem, then you can
- have a clean install of mediawiki using the same database(do backup), or
- have a clean install of mediawiki then change the database name to
the old one in LocalSetting.php and run maintenance/update.php

fredbaud, nothing about extension/require can cause 404.
