How can I add a group manually?

One thing I spot on the extension page:
"Extensions were tested on MediaWiki versions 1.6.3, 1.6.5 and 1.9.3.
I do not guarantee I will works on higher versions."

What version are you using? The latest version(s) are slightly
different from 1.9 in terms of userrights, so the extension might not
work (I'm not entirely sure what's changed, but I know some stuff
has).

The standard way of managing groups is by editing the
LocalSettings.php file and adding lines like:

$wgGroupPermissions['group']['permission']=true;

And then adding and removing people from groups using Special:Userrights.
