[Mediawiki-l] Embed a template in the Upload File page

You can change the boilerplate text for your wiki by looking at Special 
Pages and then System Messages (aka, Special:Allmessages).  Search for 
the text you want to change and click on the name in the left column. 
That will open the edit page.

I don't know if it will properly handle a template, though.  Give it a try.
