The various Wikimedia projects add to the edit toolbar as desired
using JavaScript; for example, see
http://en.wikipedia.org/wiki/MediaWiki:Common.js - search for "Extra
toolbar options."

You can use this in the MediaWiki:Common.js page on your own wiki.
