I think he means a _template_, not an extension. That's what's indicated by
the syntax he used:

Something like this. Instructions for Windows,
adapt if using Linux.
* Create a folder: C:\Inetpub\wwwroot\wiki\uploads
* Enable write permission for the user IUSR_MACHINE.
* Change these lines in LocalSetting.php:
$wgUploadPath = "$wgScriptPath/uploads";
$wgUploadDirectory = "$IP/uploads";
* And a little farther:
$wgEnableUploads = true;
I created an extension {{Download|filename|description}} that makes the link
and puts a download icon after it.
