This seems to be a common issue within a windows environment.
My first crack at mediawiki was using a WAMP setup (not my idea) and I was
always plagued with issues like this.
Check your apache error.log and then google what you see there, that's a
good start.
Check your php error logs as well. In my experience there were some issues
with php that never quite got resolved, despite going through hundreds of
bug reports and attempting several fixes.
How did I solve the problem? I switched to Linux and I've been running an
error free server ever since.
