Is there a WMF test wiki where I could play around with Flagged Revisions without breaking anything? I'm interested in especially in the configurations used on the major Wikipedias. Alternatively, is there someplace where the configurations used are are explained? My understanding is there are some differences in the usage patterns between de.wp and en.wp, for instance.

I don't think that there is one, but in practice to test Flagged Revisions i use the version installed in the Hebrew Wikisource. Maybe you can propose installing it in the Wikisource in your language. A Wikisource project is a pretty natural place for installing and testing this extension, so it shouldn't be hard to convince the community there to try it.
It also seems that a few weird bugs that affected Flagged Revisions were fixed in the 1.17 upgrade, so now is a pretty good time to test it.
