What that generally means is that one of the squids is offline. At the moment this is happening with annoying frequency as we've had some problems getting log rotation to work properly on the new machines.
Squid is brain-dead and will *crash* if its log file reaches 2 gigabytes (which happens fairly frequently when you get our level of traffic).
