I have a tendency to forget to fill out the projects/tags field when filing new Maniphest tasks. The bold red "(no projects)" IRC notification usually tips me off to my screw-up. Moving the "Tags" input above the "Description" field could help, maybe.
I haven't looked at how common filing tasks without projects is in general. Phabricator is very streamlined and focused on sane user behavior for the most part, but I've found the projects/tags input in Maniphest to be one of its weaker areas. You have to know that tasks related to "transclusion" are often associated with "MediaWiki-Templates". Finding and making that association is difficult currently.
