Looking at the patch, it seems to me that a better solution would handle both NOINDEX *and* NOFOLLOW.
Please note that the patch is about the rel="nofollow" attribute of <a> tags (specifically, external links), _not_ the like-named value of the <meta name="robots"> tag. You seem to be talking about the latter.
