The most common issue right now is you need to have title=Special:OAuth/whatever as a signed parameter, since MediaWiki adds that in.

Most WMF users should have accounts on Meta, and I would guess most Programs/Grants users would as well, since Meta is where grantmaking and program eval stuff tends to happen.
