Interestingly, I've just been through a similar thing.  All you need to do is to send a mail to the OTRS team.  See http://commons.wikimedia.org/wiki/Commons:OTRS - you cut and paste the text in the grey box on that page, change as required, and send.  You won't need hectic documentation like affidavits etc, just say who you are and what license you released it under, and all should be good.

My story: there was a request on the Kraft Foods page for a picture of the Kraft Foods Headquarters.  I got our global social media manager to source a picture and release it under a free license, but after making it to the Kraft Foods page, it actually got deleted before we noticed.  One email and a few days wait, and it was back up again.

I am able to supply an SAPS letter, Affidavit, and photograph of the license.
