On a related subject, in the term (as in, probably not this fiscal year)
something that I would like to see is an update of the Meta main page to
make finding a variety of commonly-requested resources easier, and to
change links from rarely used or little-maintained pages to pages that are
more actively maintained. The refresh to the Meta main page could be the
subject of a Project Grant for a community member with design skills; it
should be primarily a community initiative rather than a WMF one, but
supportive input from WMF would be helpful if and when someone gets around
to doing that. Among other advantages of this refresh might be improving
the "findability" of portals that are requested and/or used a lot by the
community.

As to your newest follow-up question:

the redesigned Evaluations portal with the Communications Center and the
resources on Outreach wiki?
The Evaluation portal [1] and the Communications Resource Center [2] are
two different portals on Meta Wikimedia that serve different needs and
audiences. The resources on Outreach Wikimedia are, in turn, organized in
different portals as well, on a different Wikimedia project. All of these
are known issues, and this is why we proposed to develop the Wikimedia
Resource Center, a hub that can improve findability of resources.
To read more about the goals of the Wikimedia Resource Center, please go to
the Wikimedia Foundation Annual Plan [3]. To learn more about the goals of
the Evaluation portal, please visit the section "Introduction to
Evaluation" in said portal [4].

I understand your desire for a comprehensive update. The portals you have
mentioned are very different, being aimed at different audiences and
different use cases. In fact, I'd say they're totally unrelated projects
that just happen to have had the word "portal" put in their title; I
caution against seeing too much meaning in such confluences of
terminology.
Given that, I encourage you to reach out to the
respective owners of the
individual projects to get more information about them, rather than
seeking
a unified report.
Regarding the "Discovery portal", I believe you were referring to
www.wikipedia.org. There is comprehensive documentation on
MediaWiki.org
