It seems very simple to me. The event in Boston is the International Wikimania Conference (or World Wikimania Conference or Worldwide...),
Actually, the title is "Wikimania [2006], the [second annual] International Wikimedia Conference", which leaves room for many other Wikimedia Conferences.
the event in Holland is the Dutch Wikimania Conference and the one in China is the Chinese Wikimania Conference. Would that not work?
Those events are currently called the Dutch Wikimedia Conference and (in Chinese) the Chinese Wikimedia Conference, which seems fine to me. But all big international groups do something this -- with national and regional meetings. There's no way to get all 100,000 contributors together in one place (just imagine... :-)
O'Reilly's Foo Camp and Bar Camps are a good example. Only a few hundred people can fit into / make it to Foo Camp, which is the big event, so there are Bar Camps in various locations, at various times, for everyone else.
