I know it's get complicated very quickly! I suppose though that our "storyboard" is already on our WLM website, like here on the Dutch one:

The storyboard on Commons should just be a quick reminder of that one, so the steps should be

1) Go find a monument in your neighborhood and take a picture
2) Keep track of the numbers of the monuments you take pictures of (could be part of step one, if you are using some map like the one showing "these monuments still don't have a picture on commons")
3) Login to Commons
4) Upload the picture on Commons
5) No luck doing it this way? Contact us on the website (option one) or use Flickr (frowned upon, since we *want* people to learn how to use Commons, and this involves hand work by volunteers)

If the user manages to jump all of these hurdles, it would be great for them to receive a "Thank you for participating and uploading a monument!" message.

Note that the original Dutch upload form from 2010 is still on Commons here:

We have been discussing some issues as well.

For instance, if the user clicks on the list and jumps to the pre-filled
form, but it's logged out (it has to be logged in to upload images),
that can be a mess....

...so we need to explain the user how to use the list to pre-fill the
form... and I like he idea of the storyboard, I'm just not sure we have
manpower enough to do it...

And I'm also not sure we can use a different storyboard for campaigns...
