WiDaR (which is used by the Wikidata game, among others) creates (almost) empty items when it I'd used to merge  two items with different descriptions in some language. In that case all other stuff is merged, but the item that should become a redirect is left behind with only the problematic description and nothing else. See Q19921058 for  case where this issue created such an item after a recent merge attempt of mine.
