We generally loan only to members and the equipment is insured, so we don't have a "sign out form". members are aware of their responsibilities and the risk is low. When it comes to more expensive pieces of equipment, however, we use the agreement attached. It's not CC-BY-SA because it's a derivative of one we get free from one of our professional memberships, but maybe it will help you.
