I'm not seeing the ToC at all on the page linked to in the bug, and anyway it seems like an edge-case (99.9% of pages readers will see will have 1st & 2nd level headings) that we don't necessarily need to find a fix for before pushing ToC to stable.

Maryana what would you advise doing in this situation? Since we are
pushing this to stable it's probably worth making a call on how to
