This was _so_ confusing to me as a newbie. There are a lot of
references to common.js but only a few that explain that you don't
create a file in the skins directory, but just create a page in your
wiki by that name.
So, if your mainpage URL is like this:
http://my.wiki.host/wiki/index.php/Main_Page
Create a new page with this name:
http://my.wiki.host/wiki/index.php/MediaWiki:Common.js
Paste the java script into this page.

i dont have common.js file in my skin directory (wiki\skins\common).
so i created a file common.js with "extra toolbar option"
but it is not working common.js file contain only the if statement from
the
