Looks very nice. Just a few things<br>*please add a field " other comments" to leave some room to add information about allergies (food and other (i know there is a food thingy, but someone might, for instance, be allergic to rubber)), need of medicins, or something else what you didnt think of. (and there is always such a thing ;) )
<br>*If there are hard words like ATM, CTOY etc in the form, just link them to Wikipedia :-)<br>*on giving your wikimedia-account, you can select commons as language, but that is not very logical. It might make sense to add Commons and other language independent projects too to the menu of Wikipedia, Wikisource etc.
<br>*Might it make sense to add a field "organization" if the person is not from Wikimedia?<br>*Birthday: add whether it is ddmmyyyy or yyyymmdd (although you find out after typing it, it looks better imho.<br>*Would it be possible to keep the fiels as they are when switching from language? (as someone who is not understanding what is said, can switch from language to see whether he understands it better there). I guess this will be hard, but if its easy, it would be nice :)
