Looks very nice. Just a few things
*please add a field " other comments" to leave some room to add information about allergies (food and other (i know there is a food thingy, but someone might, for instance, be allergic to rubber)), need of medicins, or something else what you didnt think of. (and there is always such a thing ;) )
*If there are hard words like ATM, CTOY etc in the form, just link them to Wikipedia :-)
*on giving your wikimedia-account, you can select commons as language, but that is not very logical. It might make sense to add Commons and other language independent projects too to the menu of Wikipedia, Wikisource etc.
*Might it make sense to add a field "organization" if the person is not from Wikimedia?
*Birthday: add whether it is ddmmyyyy or yyyymmdd (although you find out after typing it, it looks better imho.
*Would it be possible to keep the fiels as they are when switching from language? (as someone who is not understanding what is said, can switch from language to see whether he understands it better there). I guess this will be hard, but if its easy, it would be nice :)
*Maybe it would be usefull to add a check on the linguistic part "I dont need translation" or maybe "please try to find a translator for me". (maybe both)
*Paper receipt: it is unclear whether this receipt will be sent through normal mail, so what address should be filled in. Also it looks like you automaticly receive a digital receipt, and you dont need to fill in an emailaddress when requesting a digital receipt, is that correct?

I like it a lot, very professional.
A few suggestions that haven't been pointed out by my fellow friends:
* Add a []Don't care checkbox in 'Preferred Room', I personally don't
care if I'm with other person or 7.
* Under receipt : Mailing Address, the field should be a textbox to put
the address not a textfield.
The rest, I think it looks really good, very professional, maybe if you
template it I might be able to translate it to spanish (the final
version) if you wish.
